Good Faith Estimate
Your Right to Know What to Expect
Under the No Surprises Act (effective January 1, 2022), you have the right to receive a Good Faith Estimate of the expected cost of your care before your first appointment, or at any point during your treatment.
This right applies to uninsured clients and clients who choose to pay out-of-pocket rather than use their insurance. It ensures you can make informed decisions about your care without unexpected costs.
Request Anytime
You can request a written Good Faith Estimate before scheduling your first session or at any point during your care. No justification required.
Timely Delivery
You will receive your estimate within 3 business days of scheduling — or within 1 business day if your appointment is less than 3 days away.
Dispute Protection
If you receive a bill that is $400 or more above your Good Faith Estimate, you have the right to dispute that bill through a federal dispute process.
No Obligation
Receiving a Good Faith Estimate does not require you to proceed with services. It is informational only and does not constitute a contract.
Your written estimate will include:
- Provider name, address, and National Provider Identifier (NPI)
- A description of the services planned and the expected diagnosis or presenting concerns
- The service code (CPT code) for each type of session
- The expected number of sessions and estimated cost per session
- An estimated total cost over the anticipated course of treatment
- Information about your right to dispute a bill if it significantly exceeds the estimate

Please note: A Good Faith Estimate is not a contract and does not guarantee the final cost of your care. Actual costs may be higher or lower depending on the services provided. The estimate reflects expected charges based on information available at the time it is prepared.
This notice applies to uninsured clients and self-pay clients. If you are using insurance, your costs will be determined by your plan's benefits, copays, and deductibles. Clients seen through Headway receive separate billing disclosures through that platform.
